About the role

This role supports seniors living with dementia across NTUC Health's community care services, including senior day care and home care settings. You will work with care teams, caregivers and community partners to assess needs, plan activities and build staff capability in dementia care. It suits registered nurses or occupational therapists with several years of experience in mental health, geriatric or dementia care.

What you'll do

Carry out comprehensive assessments and advise the care team and caregivers on how cognitive, physical and social impairment affects daily living activities
Recommend activity prescriptions for clients with dementia
Train and support staff and caregivers on engagement strategies for people at different stages of dementia, in individual or group sessions and specific programmes

Review staff competency in facilitating activities for clients at various dementia stages

Plan group activity programmes in day care and adjust schedules to suit each centre's needs

Advise on changes to the environment in day care or home settings to improve client participation in daily activities

Assess behaviours of concern and recommend suitable strategies to staff and caregivers

Support and train staff on applying behaviour management strategies at centre level

Document professional input and recommendations electronically and share them with care staff and caregivers

Develop training and educational materials and conduct training or sharing sessions for the care team and public events

Conduct holistic assessments to identify needs, coordinate services, educate and advocate for clients and their support systems

Coordinate with clients, caregivers and key service providers so care plans are developed appropriately

Formulate care plans and coordinate care across the continuum of services, identifying suitable providers and facilities and using resources in a timely and cost-effective way

Facilitate or lead interdisciplinary case discussions within senior day care or with community partners

Develop and review programmes and evaluate their effectiveness to support continuous improvement

Provide consultancy on centre design and environmental modification to create dementia-friendly spaces

Provide consultancy on special projects related to dementia and new programme studies or research

Requirements

Registered Nurse registered with the Singapore Nursing Board, or Occupational Therapist registered with the Allied Health Professional Councils
Advanced Diploma in Gerontology, Medical & Surgical, Mental Health or any discipline, or Specialist Diploma in Geriatric or Community Nursing (for nurses)
Diploma or Bachelor of Science in Nursing (for nurses)
Diploma or Bachelor of Science in Occupational Therapy (for occupational therapists)
At least 5 years of experience with mental health, geriatric or dementia populations in any setting
