Senior / Assistant Senior Social Worker (CREST)

About the role

You will be part of a community outreach team under the Community Resource, Engagement and Support Team (CREST), raising awareness of mental health conditions and dementia, identifying at-risk individuals early, providing emotional support to clients and caregivers, and linking them to health and social care services. You will provide clinical leadership and supervision to social workers, oversee case management and documentation, and ensure the CREST programme follows the Tiered Care Model. You will also plan, coordinate and evaluate outreach and public education programmes, track programme data and prepare reports for funders. In addition, you will engage community stakeholders to build partnerships and support clients and caregivers.

Requirements

• Bachelor of Social Work or Postgraduate Diploma in Social Work from an accredited institution
• Registration with the Singapore Association of Social Workers (SASW) preferred
• 6 to 8 years of relevant experience in healthcare or community care settings
• 3 to 5 years of direct clinical practice with individuals with mental health and/or dementia-related conditions
• At least 3 years of supervisory experience
• Strong proficiency in written and spoken English
