You will assess, develop, implement and evaluate clinical practice and standards of care for patients needing community care and supportive or palliative care. You will work closely with nursing staff, physicians, the inter-disciplinary team and community partners to provide optimal support to patients. You will act as a clinical resource person to nursing staff and manage a caseload in Neuroscience. You will adopt evidence-based clinical practice and teaching programmes, and provide resources and patient education to patients, families and the public.
