About the role
You will join the Precinct Planning & Development team at NUHS Population Health & Community Care, working on Healthy Precincts, a place-based approach to community health in western Singapore. Reporting to a Manager or Senior Manager, you will help plan and implement precinct-level strategies and coordinate multi-stakeholder initiatives. This suits someone who enjoys working with diverse partners, tackling complex problems, and making a difference at community level.
What you'll do
Conduct population, community and precinct analysis to identify health needs and opportunities
Develop and support Community Health Plans
Translate population health priorities into practical initiatives and workplans for assigned precincts
Support development of theories of change, logic models and implementation plans for community-based interventions
Build positive working relationships with healthcare providers, social service agencies, grassroots organisations, community groups and government partners
Drive and/or support multi-stakeholder workgroups and partnership platforms
Engage stakeholders to strengthen collaboration and collective action towards shared population health and community goals
Participate in resident engagement and co-creation activities to keep initiatives responsive to local needs
Lead planning and implementation of Healthy Precinct initiatives across assigned precincts with multiple stakeholders and agencies
Monitor project milestones, deliverables and risks, and support follow-up actions
Track and report project progress and outcomes
Gather, analyse and interpret population health, programme and community data
Track programme implementation and outcomes to support continuous improvement
Develop dashboards, reports and presentations for internal and external stakeholders
Prepare meeting materials, presentations, reports and briefing papers for Healthy Precinct platforms, senior management and/or partner organisations
Document key discussions, decisions, action items and follow-up plans
Facilitate effective communication and coordination across multiple stakeholders

Requirements
Bachelor's degree in any field
At least 3 years of relevant experience in stakeholder engagement, programme management, partnership development, community development, policy, strategy or project management
Strong interpersonal, facilitation and relationship-building skills
Strong analytical, problem-solving and organisational abilities
Excellent written, verbal and presentation skills
Ability to work collaboratively across multiple stakeholders and disciplines
