About the role

NUHS@Home is a hospital-at-home programme delivering acute hospital-level care to patients in their own homes. The service sits under the Division of Advanced Internal Medicine, Department of Medicine, National University Hospital, and works through multi-disciplinary teams supported by remote monitoring and video consultation. This role suits a doctor who is agile, innovative and proactive, and who wants to bring medical services out of the hospital and into patients' homes.

What you'll do

Provide clinical leadership and oversight to the NUHS@Home team, including junior doctors, nurses, pharmacists and allied health professionals
Conduct patient assessments, develop and implement care plans, and manage acute illnesses on an ongoing basis

Take part in daily virtual team rounds with the interdisciplinary team to review patient progress and update care plans

Communicate with patients and their families so they understand the care plan and are comfortable with the care provided

Use technology enablers such as remote patient monitoring and video consultation to support patient care and safety

Conduct home visits and point of care investigations as required

Ensure compliance with NUHS@Home policies and procedures and keep accurate, complete medical records

Participate in research and quality improvement initiatives to improve the programme

Requirements

Basic medical and postgraduate qualifications from accredited medical universities or schools registrable with the Singapore Medical Council
Successfully registered with the Singapore Medical Council and accredited by the Specialist Accreditation Board, Ministry of Health
Completed advanced specialty training in Internal Medicine, Emergency Medicine or Family Medicine
Willing to embrace new technologies and innovative ways of delivering care
