When
You will provide integrated person-centred health and social care to residents in the SingHealth region. You will support Senior Associate Wellbeing Coordinators and Wellbeing Coordinators in carrying out care plan interventions. Your duties include monitoring residents' wellbeing through regular check-ins, coordinating care with stakeholders, and maintaining updated documentation. You will also assist in screening residents for health and social care needs and share information on community resources. You will support community outreach and engagement efforts.
