When
You will support the stroke care team in screening patients and caregivers for symptoms and needs in the clinic. You will build strong relationships with patients, families, and partners, and act as the main contact for care coordination. Your duties include administering distress thermometer screening, basic vital signs, and questionnaires, and escalating care when needed. You will also collate screening forms, maintain databases, follow up with patients by phone or home visits, and help educate families on self-management and healthy lifestyles. You may also support community outreach efforts.
