When
You will support clients with or at risk of mental health issues and dementia by linking them to suitable support and resources. You will monitor and support clients in the community as assigned by the lead. You will plan and conduct outreach to raise mental health and dementia awareness and promote early identification. You will also work with regional health services, hospitals, volunteers, grassroots, schools and internal departments on activities and programmes for seniors. Occasionally, you will support weekend outreach events.
