About the role
This role supports Healthier SG work in GP clinics, helping patients with diabetes and other chronic conditions access screening, follow-up care and community services. You will work closely with general practitioners, clinic assistants and a PCN nurse, and also connect patients to social and community partners. It suits someone with frontline or customer service experience who is organised, comfortable with Microsoft Office and keen on community care.
What you'll do
- Support Healthier SG-related work at GP clinics
- Liaise with service providers to arrange annual eye and foot screening deployments for diabetes patients at GP clinics and community nodes
- Schedule patients' appointments for ancillary services, call them with reminders and collect payment during deployments
- Submit a reconciliation report after each deployment
