About the role

You will join an integrated care team at a Senior Day Care Centre, delivering person-centred care for seniors, including those with dementia. The role focuses on promoting independence and maintaining cognitive and physical function, and you will assist the Community Care Manager in keeping centre operations running smoothly. It suits someone with a care or social service background who enjoys leading a team and working closely with seniors and their families.

What you'll do

Supervise and support the direct care team in delivering Assisted Daily Living tasks
Coordinate and carry out each senior's individual care plan
Keep the centre safe and clean
Engage seniors regularly to monitor their cognition and functional well-being

Work with healthcare professionals to track and report concerns

Implement programmes according to guidelines and gather feedback to improve them

Tailor centre-based programmes, including intergenerational and volunteer programmes, within approved guidelines

Help prepare documentation for incident reporting

Support next-of-kin and caregivers with information and guidance on client care

Maintain centre amenities and arrange client transportation

Monitor inventory and submit requests to Procurement or Facilities

Liaise with vendors, technicians and facilities management

Support trialling and evaluation of new equipment or technology, including user feedback

Supervise and support staff in executing crisis response SOPs

Plan staff rosters and manage day-to-day changes

Help the Community Care Manager prepare and gather documents for internal and external audits

Supervise and support the rollout of new non-clinical SOPs

Supervise activities delivered by volunteers and gather feedback for re-engagement

Schedule training courses and on-the-job training for staff

Conduct on-the-job training and assessments

Good to know

Minimally a Diploma, preferably in social service or a related discipline
Experience managing and leading teams is an added advantage
Working experience in hospitality or community support is a plus

Requirements

Minimally a Diploma, preferably in social service or a related discipline
Experience managing and leading teams is an added advantage
Strong interpersonal and communication skills, both verbal and written
Able to work independently and as a team player with all levels
Proficient in Google Suite Applications
Growth mindset and commitment to continuous learning
