When
You will deliver integrated health and social care for mothers and children, focusing on antenatal and postnatal care, developmental-centred parenting, parent-child interaction and family well-being. As the keyworker, you will support the mother-child dyad and family through home visits and clinic touchpoints, and coordinate services between families and healthcare and social service systems. You will conduct mental wellness screenings, nutritional assessments and child development screenings, and work with a trans-disciplinary team to make referrals and family-centred recommendations. You will also partner with community and social service agencies or hospital specialists to provide support for clients and families.
