About the role

You will deliver nursing care at a senior day care centre, managing assessments, care plans and evaluations for elderly clients, and supporting their families. The role sits within a community health team and suits a registered nurse who is comfortable working both independently and alongside colleagues. You will also help maintain clinical standards and guide care staff.

Follow approved nursing practice standards

Assess needs and provide geriatric and dementia nursing care

Conduct pre-assessments for new clients referred by AIC

Ensure client enrolment follows company procedures

Prepare and submit documentation to relevant government bodies

Work with the health care team to assess, plan, implement and review client conditions

Monitor client progress and adjust care plans

Offer health promotion and counselling to clients and families

Carry out nursing tasks such as medication administration and wound dressing

Supervise and manage the performance of nursing and care staff

Coordinate and monitor clinical and service quality indicators

Perform CPR, AED and first aid during emergencies

Support health promotion activities at the centre

Take part in clinical service projects and infection control campaigns

Participate in training and development of care staff

Requirements

Registered Nurse with Singapore Nursing Board
Degree or Diploma in Nursing
Minimum 3 years of relevant work experience preferred
Advanced or Specialist Diploma in Geriatric or Community Nursing is an advantage
Strong sense of integrity
Pleasant and cheerful disposition with good communication and interpersonal skills
