When
You will oversee and coordinate the SingHealth Healthier SG team's service delivery and resource deployment within the Community of Care, reviewing services and providing governance on operational issues. You will work with internal and external stakeholders to formulate, implement and review strategies for value-based care in the eastern region, and drive policies for population enrolment. You will engage regional GPs and Primary Care Networks to adopt care protocols, programmes, IT systems and financing arrangements, and implement integrated community care teams. You will act as the primary liaison for RHS matters with MOH and AIC, establish planning, governance and risk management frameworks, and develop key performance indicators to monitor programme effectiveness. You will also build partnerships with community partners and contribute to manpower development by developing training programmes in population health.
