About the role
This role focuses on delivering care assessments and helping residents set and achieve health goals. You will carry out lifestyle interventions aimed at lowering health risks, assisting with chronic disease self-management, and enhancing quality of life through lasting behaviour change. Working with the Community Health Team, you will contribute to holistic care planning and effective service delivery.
What you'll do
- Evaluate residents' health status and risk factors using subjective and objective methods
- Guide residents and stakeholders through collaborative goal-setting with the S.M.A.R.T. framework
- Create individualised health coaching plans to support residents' wellness goals
- Help residents overcome lifestyle and behavioural change obstacles
- Monitor and record resident progress and health outcomes
