About the role

NTUC Health is a major eldercare and community care provider in Singapore, and the Care Transition Office helps seniors move smoothly between hospital care and the organisation's services. This manager role leads the setup of a centralised assessment function and improves how care is coordinated across teams. It suits an experienced clinical or social work professional with leadership and process improvement background.

What you'll do

Set up operational guidelines for centralised assessment support (InterRAI CU) across Senior Care Centres, Home Care and Community Social Services
Carry out assessments directly and review workload volume to plan future team growth (up to 4 assessors)
Be ready to become a master trainer for InterRAI under a train-the-trainer model

Improve internal onboarding and transfer processes with Care Coordinators to standardise handoffs and remove transition bottlenecks

Help design, pilot and evaluate journey-based integration models that combine multidisciplinary care plans into one client record

Ensure assessment workflows follow AIC guidelines, MOH regulatory standards and internal clinical and social governance frameworks

In a later phase, recruit, onboard, train and supervise junior Care Assessors as demand grows

Team supervision is planned for Phase 2 as assessment demand expands

Requirements

Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy or Healthcare Management
At least 5 years of clinical, social work or care assessment experience in eldercare or community health
Demonstrated project or team leadership
Certified InterRAI Assessor, or eligible and willing to undergo immediate certification
Strong process optimisation and change management skills
Strong interpersonal and engagement skills
