When
You will provide clinical and community social work support to mothers and young children from early conception through the first few years, within a multi-disciplinary outpatient setting. Duties include intake interviews, risk assessments, case triage and assignment, home visits, and referrals to community partners or financial assistance. You will also supervise, coach and give social work input to a multi-disciplinary team, and keep case documentation promptly completed and filed.
