Provide psychosocial and financial assessments, casework and interventions for residents in a nursing home. Screen referrals from AIC and coordinate onsite assessments with nursing teams and stakeholders. Work with the multi-disciplinary team and next of kin to support residents' transition and care planning. Assess eligibility for subsidies and financial assistance schemes, and maintain residents' records. Supervise case management executives and care concierge in pre-admission processes.
