Provide individuals, families, caregivers and vulnerable populations with psychosocial support to cope with chronic and acute illnesses. Duties include case management, assessment and formulation of intervention plans, care coordination and supportive counselling. You will also provide patient education, information and referrals to other community social services, and take part in research and programme development.
