About the role
You will provide interim in-home care for clients who have just been discharged, before their longer-term care arrangements are ready. Working with NTUC Health's home care team, you will support clients with personal care, hygiene and daily living needs. This role suits someone who is patient, observant and comfortable working in a fast-paced home care setting.
What you'll do
Help clients with personal hygiene, including bathing or assisting with bathing for those who are unwell, bedridden or have mobility issues
Assist with changing clothes, undergarments, continence aids and soiled sheets

Lift and reposition bed-bound clients every two hours and conduct elimination rounds for non-ambulant clients
Perform simple dry wound dressing and bandaging
Assist with feeding, including oral and nasogastric tube feeding
Help administer medications and give medication reminders
Provide companionship and recreational activities
Carry out mentally stimulating activities
Guide clients through simple physical maintenance exercises
Escort clients to medical appointments
Attend to client needs and relay requests to caregivers or Care Coordinators
Monitor and record vital signs, and report any abnormalities
Identify and fix basic home hazards to prevent falls or slips, such as wet floors or untidy walkways
Keep the client's passageways, bedroom, living room and toilet dry and clean by sweeping and mopping
Keep the kitchen stove and table clean after cooking done by Nursing Aides
Help wash plates and pots after meals or after cooking done by Nursing Aides
Help wash the client's clothes and hang them to dry indoors where washing facilities are available
Follow standard precautions and hand hygiene when in contact with clients and their surroundings
Educate clients and relatives on infection control practices
Assist the ICS Care Coordinator with enquiries and feedback from referral sources and home care clients
Liaise closely with care team members, family members and external partners on client care needs
Perform other care duties assigned by the Operations Manager or Nurse Manager
Good to know
Locally trained candidates should have a GCE N Levels certificate or completed one of these: WSQ Higher Certificate or Advanced Certificate in Healthcare Support (Nursing Care), or an ITE-approved Training Centre Healthcare (Home Care) programme
Foreign-trained candidates should hold a nursing degree from their home country and a valid practising license there

Locally trained: GCE N Levels certificate or completion of WSQ Higher Certificate or Advanced Certificate in Healthcare Support (Nursing Care), or ITE-approved Training Centre Healthcare (Home Care) programme
Foreign-trained: nursing degree from home country and valid practising license in home country
