About the role
Join the NUHS@Home team at the National University Health System, where you will help shape a Hospital-at-Home programme that brings inpatient-level care into patients' homes. This role suits an experienced clinical pharmacist who wants to work at the top of their licence and influence how home-based hospital care is delivered. The position can be taken up on a full-time or part-time basis.
What you'll do
Take part in daily interdisciplinary rounds with doctors, nurses and allied health colleagues to build and carry out patient care plans in a home-based hospital setting
Run daily virtual ward rounds with patients to advance pharmacological care and improve outcomes
Apply inpatient care knowledge to manage complex cases across many specialties
Review and improve medication regimens to boost adherence and reduce adverse effects, while helping transform clinical care models
Give expert advice on medication management to maintain high standards of safety and efficacy
Educate patients so they can manage their own therapies at home
Support clinical governance, strategic planning, and the writing and rollout of protocols and guidelines for hospital-at-home care
Keep learning and developing professionally to stay at the forefront of healthcare innovation
Provide general clinical pharmacy services, including accurate medication records, medication reconciliation and adherence support
Identify patient-specific medical problems and recommend, implement and manage pharmacotherapy, including reviewing, counselling, dispensing and supplying medicines
Monitor and adjust drug therapy, prevent or resolve drug-related problems, and work with other providers to improve outcomes
Promote and monitor adherence to prescribing guidelines and drug use protocols
Give professional, evidence-based drug information to patients and the care team
Work with patients, caregivers and providers to ensure safe, cost-effective and timely distribution and quality use of medicines
Communicate with patients and caregivers individually or in groups so they understand their prescribed drugs and cooperate with therapy
Document clinical activities as set out in the competency development framework or department guidelines
Lead or serve on departmental and institutional clinical practice committees as assigned
Plan, implement and deliver education and training programmes for entry-level health professionals and the public
Help develop or publish high-quality patient education materials and articles for professional education
Lead the development of pharmacy professional standards, competency frameworks and assessments for future clinical pharmacists in this telehealth service
Lead and contribute to the continuing professional development strategy
Lead, start and take part in clinical research alone or with other healthcare disciplines
Publish in peer-reviewed journals or present findings at local and overseas scientific meetings

Requirements
MSc in Clinical Pharmacy, PharmD or Board Certification
Completed a Collaborative Prescribing Course within the last 2 years
At least 5 years of clinical experience, ideally in inpatient care
Registered with the Singapore Pharmacy Council
Extensive and current knowledge of pharmaceuticals, clinical practice guidelines and protocols
Strong leadership qualities
