About the role
This role is for a doctor to join a hospital clinical service in Singapore, working across inpatient, community health and nursing home settings as part of a multidisciplinary team. It suits a registered medical practitioner who is comfortable supervising junior doctors, teaching, and coordinating care beyond the hospital ward. You will also take on quality improvement and clinical governance work within the department.

Support the consultant in charge in overall supervision and management of medical matters for the clinical service you are attached to.
Supervise and teach Medical Officers and Family Medicine trainees, including procedures and assistance with more complex cases.
Complete discharge summaries and reply to referring doctors' letters with diagnosis and progress updates, keeping a hard copy in the case notes where applicable.
Communicate with patients, their legal guardians or designated individuals about the patient's condition; for unconscious or mentally incompetent patients without a designated person, speak with next-of-kin or responsible family members.
Explain tests and procedures to patients and relatives.
Sign death certificates in services where this applies.
Attend departmental, clinical incident and multidisciplinary meetings, and take part in teaching sessions organised by the department or campus.
Provide transitional home care for patients discharged from hospital as part of the multidisciplinary team.
Join multidisciplinary rounds and act as medical lead in those rounds.
Review all investigations done by the community health team service or other home care equivalents.
Act as medical liaison with other home care teams and with hospitals.
Review medications and appointments to check they are adequate and suitable.
Provide medical support in the community for medication and appointment reviews and patient assessments including geriatric screening, when the nurse or health coach cannot locate the patient's primary physician or the patient has none.
Train community providers in the medical aspects of patient care.
Help the nursing home systematically review patients according to Ministry of Health guidelines.
Assist the nursing home with infection control, medication management, restructured hospital outpatient clinic appointments, quality management and investigation of adverse events.
Assist the consultants in charge of the nursing home with triaging patients, advanced care planning and family conferences on end-of-life care.

MBBS from a Singapore university or equivalent
Full or Conditional Registration with the Singapore Medical Council
Valid Practising Certificate from the Singapore Medical Council
For Resident Physician: at least 3 years as a medical officer post-housemanship, or completed post-graduate year 4
For Senior Resident Physician: at least 7 years as Medical Officer post-housemanship or as Resident Physician
