Provide postnatal medical assessments and evaluations for infants during home visits. Offer anticipatory guidance and health promotion for families, and give clinical advice and support to keyworkers managing the mother-child dyad. Contribute advisory input in multi-disciplinary case discussions and coordinate care by collaborating with healthcare colleagues and making referrals to hospital specialty clinics. Work with community partners, including facilitating referrals to Child Protection Service when needed. Participate in medical teaching, training, continuing medical education and research activities, and maintain appropriate clinical service quality standards.
