Join the Integrated Community Care Provider team to support seniors and their families. You will work with internal teams and external partners to understand seniors' needs and recommend suitable services. Duties include managing client cases, assessing needs, facilitating service enrolment, organising meetings, tracking follow-ups, and preparing workflows, presentations and reports for regulators and stakeholders. On-the-job training is provided.
