About the role

Provide psychosocial and financial assessments, casework and interventions for residents in a nursing home. Work with the multi-disciplinary team to support residents' transition, care planning and discharge. Facilitate family conferences and caregiver support. Supervise and mentor medical social workers, executives and interns, and lead improvement projects.

Requirements

Degree or Post Graduate Diploma in Social Work
Registered with Singapore Association of Social Workers (SASW)
At least 3-5 years of relevant experience in intermediate and long term care settings
Experience in leading and supervising a team of medical social workers and executives
Good interpersonal and problem-solving skills
Proficient in Google Workspace
