You will provide pharmaceutical care by working with a multidisciplinary healthcare team to promote wellness, maintain health, and manage acute or chronic illness. Your duties include recommending and monitoring patient drug therapy, providing drug information, and identifying, resolving, and preventing drug-related problems. You will interact with patients, families, and other healthcare providers to ensure continuity of care and achieve desired outcomes. You will also ensure accurate and timely distribution of pharmaceuticals and provide education to patients, staff, and other healthcare practitioners.
