You will provide pharmaceutical care and drug information to patients and healthcare professionals. You will identify, resolve and prevent drug-related problems to improve patient outcomes. You will educate patients, caregivers, pharmacy students and healthcare providers. You will take part in quality improvement and evidence-based practice initiatives. You will work with internal and external stakeholders to ensure continuity of care.
