You will support medical social workers by conducting phone screenings to collect patient biodata and social information, and by contacting patients for appointment reminders and rescheduling. You will accompany medical social workers on field visits or conduct them independently, complete case recordings on time, and liaise with external agencies. You will also transcribe applications onto online referral systems, monitor and follow up on referral statuses, handle external correspondence, and attend to Medical Social Services enquiry emails. You will maintain work statistics and perform clerical duties as needed.
