About the role

You will deliver psychosocial assessment, intervention and case management for clients with complex social and healthcare needs in the community. The role sits within NTUC Health's community care services, supporting seniors and their caregivers to age in place safely and with dignity. It suits an experienced social worker who is comfortable handling high-risk cases and working alongside healthcare and community partners.

What you'll do

Carry out thorough psychosocial assessments for clients and their caregivers
Create and put into action person-centred care plans guided by clients' needs and goals
Offer counselling, emotional support, crisis intervention and practical help to clients and families
Handle complex, high-risk cases on your own
Track client progress and review intervention plans on a regular basis

Make referrals and coordinate services with hospitals, community partners, government agencies and healthcare providers

Advocate for clients' needs and their access to community resources and financial assistance schemes

Help caregivers cope with caregiving stress and challenges

Run family meetings and mediate family-related issues when needed

Educate clients and families about available social and healthcare resources

Work closely with nurses, doctors, therapists, care coordinators and community partners for integrated care

Take part in case conferences, multidisciplinary meetings and care planning discussions

Build collaborative relationships with external stakeholders and service providers

Guide, mentor and clinically supervise junior social workers, interns and support staff

Support onboarding and training of new staff

Contribute to team discussions and professional development initiatives

Keep accurate, timely and professional case documentation and reports

Ensure compliance with organisational policies, professional ethics and regulatory requirements

Prepare reports, statistics and service data when required

Participate in programme planning, service evaluation and quality improvement initiatives

Suggest ideas to improve workflows, client outcomes and service delivery

Support audits and organisational projects when required

Good to know

Degree or postgraduate qualification in Social Work recognised by the Singapore Association of Social Workers
Registered Social Worker status is preferred
Five to eight years of relevant social work experience preferred, ideally in healthcare, eldercare, community care or case management
Supervisory or mentoring experience is preferred

Requirements

Degree or postgraduate qualification in Social Work recognised by the Singapore Association of Social Workers
Registered Social Worker (RSW) preferred
Minimum 5 to 8 years of relevant social work experience, preferably in healthcare, eldercare, community care or case management
Experience managing complex cases and working with vulnerable populations
Supervisory or mentoring experience preferred
Strong case management and counselling skills
