When
You will work with residents in the community to promote preventive health and support them in taking steps towards better health. Your role involves contacting members by phone, SMS and email to walk them through their personalised health plan, assist with screening and appointment bookings, and track their progress. You will screen members for health and social care needs using questionnaires and motivational interviewing, and make referrals to internal or community partners when needed. You will also encourage members to adopt healthier lifestyles, connect them with community resources, and support data collection and reporting. Occasionally, you may help train junior Wellbeing Coordinators.
