When
You will work closely with Medical Social Workers by conducting home visits to gather collateral information and assess patients' home physical and familial environments. You will assist MSWs in interviewing and assessing patients and their family members for appropriate service referrals. You will also register all referrals received by the department, conduct intake interviews for MSWs, and perform means-testing for patients seeking subsidies for community hospital services as stipulated by the Ministry of Health.
