When
You will manage about 25 active cases per quarter, mainly of low to moderate complexity, conducting assessments, planning and interventions. You will hold case conferences with partners and attend networking and case management meetings. You will document and evaluate case outcomes, develop intervention strategies, and submit reports on time. You will also handle case referrals and be part of the Intake and Triage Team, referring clients to suitable agencies. In addition, you will network with other organisations, support community outreach initiatives, and join the programme team for group work at the FSC.
