Manage case referrals and active cases, conducting assessments, planning and interventions, and case conferences with partners. Attend networking and case management meetings, document and evaluate case outcomes, and develop intervention strategies. Refer clients to appropriate agencies and take part in the Intake & Triage Team for new intake assessments. Network with other organisations and stakeholders, conduct community needs assessments, and join the programme team for Group Work at the FSC.
