About the role
This role supports children and families who come through Family Nexus, helping them access health and social services. You will work closely with a care team in Women and Children's Services and with community partners. It suits someone who enjoys frontline engagement with families and is comfortable with documentation and follow-up work.
What you'll do
Engage children and families referred to Family Nexus and carry out relevant screening
Share basic, personalised health information with clients and their families
Help families move through the social-health system by pointing them to information and resources that fit their needs
Work with external agencies and community partners so referrals are followed up promptly
Promote mother-child dyad and MSF parenting programmes to children and families
Spot families who need subsidised services and help refer them to financial, social and health services
Keep accurate records of care delivered to clients and families
Support group health promotion activities in the clinic and in the community alongside institution partners
Help with engagement activities at the clinic, subject to supervisor or Clinic Head approval
Track, recall and reschedule referrals for clients who miss children's and women's health services, following set workflows
Coordinate care for clients and families together with the Women and Children's Services team
Follow the institution's patient care policies and procedures
Keep to International Patient Safety Goals and infection control practices
Comply with organisational work instructions, policies and safe practices
Keep the working environment clean and safe
Report unsafe practices and incidents according to NHGP quality standards
Take part in continuous quality improvement activities
Comply with the Personal Data Protection Act
Attend training courses to build new knowledge and skills
Assess your own learning needs and pursue learning as needed
Follow up on referrals to and from Family Nexus
Help track clinical indicators set for Family Nexus
Run and analyse surveys from user feedback
Assist your supervisor in planning and supporting group activities in the clinic or community
Make telephone follow-ups when needed
Support organisational projects and events
Help plan, organise and coordinate activities with your supervisor
Carry out incidental housekeeping such as keeping the workplace clean and topping up stationery and consumables
Take on any other duties assigned by your supervisor
