About the role

This role involves coordinating and managing care for patients in liver, multi-visceral and intestinal transplant programmes, including living donation and post-surgery follow-up. You will work closely with transplant physicians, pharmacists and other specialties, and the position suits someone with a healthcare or people-focused background who is comfortable with patient education and on-call duties.

What you'll do

Coordinate the evaluation process for living donation and transplantation, including preparing donors and recipients for surgery and following up with both after surgery
Register and manage patients on the National Waiting List for deceased-donor liver transplant
Review follow-up work processes to ensure efficient patient care

Provide counselling to patients, including potential living donors, and their families about the transplantation process

Educate patients on their primary physician's recommendation and the reasons behind it

Order and perform basic interpretation of diagnostic and laboratory tests

Order appropriate treatments and non-pharmacologic therapies according to hospital protocols

Provide post-transplant teaching and education to recipients and donors based on their individual level of understanding

Assist with ad hoc patient complaints and care arrangements using critical thinking and communication with relevant stakeholders

Assist in outpatient follow-up in the Transplant Clinic alongside the transplant physician and pharmacist

Reinforce the importance of pre- and post-transplant follow-up

Work with various transplant specialties to ensure all clinical instructions are tracked, monitored and followed up

Collate data and manage the various database systems used by the programme

Work closely with stakeholders to ensure accurate data collection and records

Collaborate with stakeholders to support defined clinical needs

Assist in research activities involving liver transplantation

Participate in organising and delivering transplant education, patient support activities and community outreach events

Handle ad hoc duties as assigned

Good to know

On-call duties are required

Requirements

Degree in Nursing, Life Science, Psychology or Health Science
Willing to perform on-call duties
Experience in healthcare or other people-centric roles
Strong communication, teamwork and patient-education skills
