About the role

This role sits within NTUC Health's Integrated Home Health Programme and Community Social Services, supporting seniors with end-stage illnesses and their caregivers in the community. You will assess care needs and provide interventions that help seniors keep their dignity, comfort and quality of life, while guiding families through caregiving, end-of-life planning and bereavement. It suits a registered social worker with community healthcare or palliative care experience who can work independently.

What you'll do

Conduct assessments, planning and interventions for cases together with the programme lead
Engage clients and build rapport

Develop, implement and review individual care plans and intervention strategies

Carry out needs assessments for clients and their caregivers

Advocate for clients' needs

Coordinate services with other community resources on clients' behalf

Handle case documentation

Manage case referrals

Conduct and take part in case conferences and multi-disciplinary team meetings

Monitor delivery of coordinated services

Document and evaluate case outcomes

Apply a case management approach, working with internal business units and other agencies or specialists on multi-disciplinary assessments and monitoring

Refer and coordinate care services so clients can continue living in the community

Share information on assistance schemes and care options so caregivers can make informed decisions about care and future planning

Support discussions on advance care planning and end-of-life preferences

Maintain accurate records and update progress reports and BumbleBee on time

Raise awareness and facilitate advance care planning for seniors

Conduct community outreach activities

Take on other ad-hoc assignments

Requirements

Graduate Diploma or Bachelor's Degree in Social Work
Registered Social Worker with SASW
At least 2 years of experience in community healthcare or palliative care settings in Singapore
1-2 years of administrative experience
Good knowledge of palliative, geriatrics and community services
Proficient in MS Office applications
