About the role
This role supports the Clinical Director and programme leads within a hospital service support area, acting as a liaison and advocate for patients across seven specialised programmes. You will focus on case management and care coordination, working with multidisciplinary teams and community partners to improve continuity of care. It suits someone organised and independent who enjoys problem solving and engaging with people at all levels.
What you'll do
Work with supervisors and functional counterparts to run and review care models and workflows
Champion case management and care coordination as contributors to programme and hospital goals
Coordinate with operations counterparts to improve programmes
Integrate care manager work with other care team workflows to improve patient experience
Assess patients' medical, psycho-social and functional needs with the care team to align with their goals
Start conversations with patients to spot potential care gaps
Encourage and guide lifestyle changes that help patients stay well in the community
Advocate advance care planning and facilitate discussions between patients and caregivers so preferences are reflected in care plans
Recommend interventions and referrals to transitional care services and community support in discussion with patients and caregivers
Follow up with patients promptly to ensure smooth care transitions
Follow workflow processes to maintain consistency, accountability and accurate documentation
Join case discussions and sharing sessions to encourage learning and updates
Apply inclusion and exclusion criteria for community and transitional care referrals, triaging referrals from the hospital messaging system and allocating patients to the community team
Update the database accurately and promptly, following up on referral assignment to the relevant care manager
Acknowledge and respond to referral sources through the hospital messaging system
Keep up to date on transitional care and community support services
Seek collaborations with internal and external transitional care and community service providers
Supervise and support care coordinators and associates in their clinical work
Take part in professional development to build knowledge and skills
Support community events run by the hospital or community partners to raise awareness and promote public health
Contribute to quality improvement efforts and research
Contribute to learning and education activities
Carry out other duties assigned by the reporting officer

Requirements
Degree or equivalent professional qualification in Nursing, Social Work or Allied Health
At least 5 years of work experience in the healthcare industry
Nursing, case management or related experience in acute and/or community settings in Singapore preferred
Experience co-developing programmes or projects with clinicians and care team members is an advantage
Knowledge of hospital specialised medical or surgical services and home medical care or community experience is an advantage
Strong team player able to interact with healthcare staff and community partners at all levels
