About the role
This role manages operational needs and service standards within a designated service support area at Alexandra Hospital, providing strategic support to Clinical Directors and Programme Leads across seven specialised programmes. It suits an experienced healthcare professional who can act as an advocate, liaison and planning partner for holistic patient care, with a focus on case management and care coordination. The incumbent may also supervise and support Care Coordinators and Care Coordinator Associates.
What you'll do
Work with Clinical Directors, Programme Leads and functional leads to develop and refine care models and workflows
Advocate for case management and care coordination as contributors to programme and hospital objectives
Track outcomes and streamline processes with operations counterparts to improve programme fidelity
Integrate and optimise care manager work with other care team workflows to improve patient experience
Understand patients' medical, psycho-social and functional needs with the multidisciplinary team
Initiate conversations with patients to identify potential care gaps
Guide patients towards positive lifestyle changes to stay healthy in the community
Advocate for advance care planning and facilitate discussions between patients and caregivers
Suggest interventions and referrals to transitional care services and community support
Follow up with patients to ensure smooth care transitions
Maintain workflow consistency, accountability and accurate documentation
Join case discussions and sharing sessions for consistent learning and updates
Triage referrals received through the hospital messaging system and allocate patients to the community team
Update databases accurately and promptly, and follow up on referral assignments
Acknowledge and respond to referral sources through the hospital messaging system
Keep updated on transitional care and community support services
Seek collaborations with internal and external service providers
Supervise and support Care Coordinators and Care Coordinator Associates
Participate in professional development programmes
Support community-based events by the hospital or community partners
Contribute to quality improvement efforts and research
Contribute to learning and education activities
Perform any other duties assigned by the Reporting Officer

Requirements
Degree or equivalent professional qualifications in Nursing, Social Work or Allied Health
Nursing, case management or related experience in acute and/or community settings in Singapore preferred
At least 6 to 8 years of work experience in the healthcare industry, with leadership experience preferred
Experience working with or co-developing programmes or projects with clinicians and care team members is an advantage
Knowledge of hospital specialised medical or surgical services and experience in home medical care and community is an advantage
Strong team player able to interact with healthcare staff and community partners at all levels
