Advocate for case management and care coordination as a contributor to programme and hospital objectives
Collaborate with operations counterparts to improve programmes
Integrate and optimise care manager work with other care team workflows to improve patient experience
Understand patients' medical, psycho-social and functional needs with the multidisciplinary team to align with their goals
Initiate conversations with patients to identify potential care gaps
Promote and guide positive lifestyle changes for patients to stay healthy in the community
