Experience Some experience About the role This role leads and manages frontline operations for a designated zone within the Home Care department, focusing on manpower optimisation and quality customer service and care. You will work closely with internal stakeholders on operations policies and engage external social and health partners to integrate clients' social and health care needs. Counsel and educate caregivers on providing appropriate care and support at home using assessment tools and care plans
Schedule and conduct regular home visits within the zone to build rapport and engage clients and caregivers
Monitor and manage leave to ensure adequate manpower and timely care delivery
Maintain staff utilisation at 90%
Recruit care staff and ensure new employees complete onboarding and training
Supervise care staff through regular supervision and coaching on service delivery
Work with the nursing team on care staff skills competencies and care standards
Perform first-contact, face-to-face or home-visit assessments to identify health and social vulnerabilities using standardised tools
Formulate care plans based on assessment outcomes to maximise functional independence and well-being of seniors
Conduct regular scheduled re-assessments (e.g., every 6 months) or as required by changing circumstances
Accurately code information using interRAI software for data quality and the national anonymised database
Coordinate within the zone to implement comprehensive support required by clients and caregivers
Provide support in complex referral management, care coordination, follow-up, service arrangement and communication for continuity of care
Monitor day-to-day service delivery, ensuring timely follow-up, coordination and escalation of issues
Support service recovery efforts for escalated cases, including home visits and coordinating follow-up actions
Engage and establish local partners from social and healthcare sectors to support clients
Lead and/or support pilot projects and workflows for ground-up initiatives
Prepare data
Monitor quality improvement initiatives to enhance care processes
Monitor initiatives aimed at driving revenue growth and increasing service utilisation
Provide secretariat support for meetings
