About the role

You will support Medical Social Workers by gathering patients' social, familial and financial information to help with service referrals. You will conduct home visits and interviews to ensure patients receive continuity of care after discharge. You will also assess patients' psychosocial adjustment in the community.

Requirements

GCE 'O'/'A' level with at least 2 years' relevant experience in social services, or Diploma preferably in Community Services or Human Services
Good team player with passion in working with people
Excellent communication skills
Proficient in Microsoft Office applications
